Abstract
Background and purpose: In the era of antibiotic resistance (AR), there is growing interest in antiseptic solutions that could provide a viable alternative for pre-operative ocular surface disinfection. We aimed to investigate the use of antiseptic eye drops in the preoperative preparation of patients undergoing repeated intravitreal injections (IVT) chronic retinal diseases. Methods: A prospective study from a single center was performed. The primary endpoint is the study of bacterial strains detected from conjunctival swab cultures in the two patient groups. Patients of Group 1 underwent at least 6 courses of topical antibiotics before IVT in the past year. Patients of Group 2 had no topical antibiotic therapy in the past 12 months. A conjunctival swab was performed at time T0 (before starting topical antiseptic treatment with 0,5% ozonized oil-based antiseptic eyedrops three times a day) and time T4 (3 days after starting treatment before surgery) in the two groups. Secondary endpoints included evaluation of the reduction of ocular surface bacterial load, treatment efficacy in terms of reducing the incidence of infection in the postoperative period, and tolerability in terms of inflammation and other ocular symptoms after administration of topical antiseptics. Results: A statistically significant reduction in microbiological load and excellent tolerability of treatment were observed in both groups. In particular, group 1 has shown a reduction of 89% of bacterial load and group 2 a reduction of 81% after 3-days treatment with ozonated oil in liposomes. In addition, following therapy, no topical adverse effects were found in either group. Conclusions: The microbiological results clearly show the antimicrobial efficacy of the antiseptic eyedrop. This study evaluated the efficacy of ozonated oil in preoperative therapy by assessing its antimicrobial activity in vivo, in terms of reducing ocular surface bacterial load, and thus promoting a greater use of antiseptic solutions over antibiotic eye drops in order to reduce the occurrence of antibiotic resistance.
