Abstract

Background: Irritable Bowel Syndrome (IBS) is a functional gastrointestinal disorder involving complex interactions between the gut, microbiota, and central nervous system. Obesity and metabolic alterations are increasingly recognized as comorbid conditions that may exacerbate gastrointestinal dysfunction through low-grade inflammation and microbial imbalance. This study evaluated the effects of an oral medical device (SBMD) containing resin, polysaccharides, and polyphenols as adjunctive therapy for IBS in overweight and obese adults. Methods: A prospective, controlled clinical study was conducted on 43 adult patients with BMI ≥ 25 Kg/m2 diagnosed with IBS according to Rome IV criteria. Participants were divided into Group A, receiving SBMD plus a low calorie low FODMAP Mediterranean diet, and Group B receiving a low calorie low FODMAP Mediterranean diet for a six-month period. Anthropometric parameters, gastrointestinal symptom scores, stool characteristics, and quality-of-life indices were assessed at baseline and post-treatment. Biomarkers of inflammation and metabolic status were also analyzed. Results: Adjunctive treatment with the SBMD resulted in significant improvement of abdominal pain, bloating, and stool consistency compared with baseline. Participants in Group A also showed modest but significant reductions in body weight and waist circumference. Inflammatory markers and self-reported quality-of-life scores improved, suggesting a beneficial impact on both gastrointestinal and metabolic profiles. No major adverse effects were observed, confirming the device’s safety and tolerability. Conclusions: This study supports the efficacy of a natural, non-systemic medical device based on resin, polysaccharides, and polyphenols as an adjunct to nutritional therapy for IBS in individuals with overweight/obesity. The findings highlight the therapeutic potential of microbiota-targeted interventions in managing functional bowel disorders associated with metabolic dysregulation.
