Abstract

The follicular patrimony of the woman undergoes a physiological and unstoppable reduction over the years, due to a phenomenon of normal ovarian aging (NOA); however, some women experience diminished ovarian reserve (DOR) much earlier than usual and become infertile prematurely, a phenomenon called premature ovarian aging (POA). The etiology of DOR is extremely varied, but in a significant percentage of women the causative factor of the syndrome remains unknown (idiopathic DOR). Furthermore, POA exposes women to an increased risk of age-related pathologies, such as neurodegenerative diseases. Therefore, the first aim of this study was to evaluate the neurological implications of POA and in particular the risk of Parkinson's. Thirty patients with DOR (defined by AMH<1.2ng/ml and/or AFC<7) and thirty women with normal ovarian reserve (defined by AMH>1.2ng/ml and/or AFC>7) have been subjected to a neurological physical examination and neurological screening tests. REM Sleep Behavior Disorder Screening Questionnaire (RBDSQ) and MSD-Unified Parkinson's Disease Rating Scale (MSD-UPDRS) showed higher mean values in the DOR group compared to control group with a statistically significant difference. A second goal of our study was to investigate the inflammatory state and immune system of these patients and to focus on new molecules, such as Semaphorin3A (Sema3A), that may could be involved in this kind of unexplained infertility. Our results showed a significant increase in the expression of the pro-inflammatory cytokines IL-6 and IL-8 and a significant reduction in the anti-inflammatory cytokine IL-10 in DOR group than in control group. About Sema 3A, a significantly higher level of Sema 3A was found in patients suffering from DOR. Furthermore, we noticed that among DOR patients who underwent in vitro fertilization techniques (IVF), Sema 3A levels correlated with indicators of good response to controlled ovarian stimulation (COS).
